
Jennifer Hoffart, MSW, PLMHP, PCMSW
Jennifer Hoffart is a Provisional Licensed Mental Health Provider (PLMHP) and Provisional Clinical Master Social Worker (PCMSW) dedicated to supporting individuals and families through every stage of life. She is passionate about building strong, authentic relationships with her clients, meeting them where they are, and empowering them to reach their fullest potential.
Jennifer believes in the importance of meeting new people and offering meaningful support during times of need. She enjoys working with individuals of all ages and backgrounds, and takes a whole-person approach to care—considering each client’s unique strengths, challenges, and goals. Jennifer strives to create a safe and welcoming environment where personal growth and healing can flourish. She is devoted to guiding clients as they uncover their strengths, overcome obstacles, and work toward a brighter, more resilient future.
Jennifer earned her Bachelor of Science in Family Studies from the University of Nebraska – Kearney. She continued her education by attaining her Licensed Practical Nurse (LPN) credentials at Central Community College – Kearney Campus and worked for several years as a Pediatric Nurse at Kearney Clinic, PC. Driven by her desire to make a broader impact, Jennifer later pursued a master’s degree in social work from the University of Nebraska–Omaha. During her graduate studies, she completed practicum experiences at Midwest Encouragement and Counseling Services in Kearney and Children’s Omaha, gaining hands-on experience in both community mental health and pediatric settings.
Jennifer’s career in mental health began as the office manager at Midwest Encouragement and Counseling Services, where she developed a deep understanding of the needs of clients and families. She later transitioned to a private practice setting, further expanding her clinical skills
and experience. Most recently, Jennifer served as an MSW Care Manager at CHI – Good Samaritan Hospital, where she provided compassionate support and care coordination to individuals facing a variety of challenges, further strengthening her expertise in social work and
mental health.
Outside of her professional life, Jennifer loves spending quality time with her family and pets, traveling to sporting events, and enjoying the outdoors whenever she can.
